Attached is a questionnaire to complete and bring with you to your initial consultation. For children under 13 years old, please use the Children’s Intake Form.

Please also bring copies of any recent blood test results or pathology reports.

As we are moving toward a paperless clinic, please feel free to return the questionnaire and pathology results via email so that they can be directly uploaded into your file.
